The EHPAD “Nou Gran Moun” (“Our Great People” in Creole) of the Capesterre Belle-Eau Hospital welcomes the elderly who are losing their independence in a modern, warm and secure setting. Our establishment places respect for the dignity and well-being of each resident at the heart of its priorities.

Our care philosophy

EHPAD Nou Gran Moun is committed to providing personalized support that respects the lifestyle, choices and history of each resident. Our approach is based on four fundamental values: dignity, respect, citizenship and responsibility. We take care to maintain social and family ties, while ensuring optimum quality of care.

Capacity

Our facilities include :

  • 80 beds in permanent accommodation
  • 5 temporary accommodation places (for stays from a few days to 90 days)
  • 2 places in temporary emergency accommodation for people over 60 who are losing their independence and no longer require active care, but whose return to or continued stay at home is no longer possible (maximum 30 days).

Who can be admitted?

The EHPAD is for people aged 60 and over who have lost their independence and require daily support and medical care.

Admission procedures

To compile your admission file :

  • Download the EHPAD admission form Cerfa N°14732*03 (direct download link) or contact the EHPAD secretariat (0590 55 90 90) to obtain the file.
  • Download the list of required documents (download link)
  • Have the attending physician complete the medical section of the file
  • Return the complete file to the CHCBE by e-mail at ehpad@chcbe-gpe.fr

A multidisciplinary admissions committee will examine your application.

Cost and financial assistance

The cost of EHPAD accommodation comprises three tariffs:

Accommodation rate: €115/day (payable by you)

Dependency rate: €/day (based on your level of autonomy as defined by the GIR)

03

Care rate (paid by the Assurance Maladie)

Temporary emergency accommodation (HTS)

Temporary emergency accommodation (HTS) consists of offering temporary accommodation in an EHPAD for a maximum of 30 days/year, financed by the Assurance Maladie (with no out-of-pocket expenses), to people meeting the following eligibility criteria:

  • people aged 60 and over,
  • when discharged from hospital or at home, in a situation of loss of autonomy,
  • in a medico-social emergency and when they are no longer in need of medical care,

This HTS system helps prepare and anticipate a safe return to the home, particularly in the event of social isolation or the sudden absence of a caregiver, and can help direct the person towards a new, permanent care structure.

It can also be used by elderly people coming from home, in the event of an urgent need for respite for the caregiver or adaptation of the elderly person’s home.

HTS differs from conventional HT in terms of the time it takes to receive care, the way it is financed, the conditions for referral and transfer, and the definition of care and admission objectives.

In fact, the HTS intervenes at a precise and punctual moment, to serve as a relay in the care of the elderly person in a very particular context.
It is not intended to respond to all the reasons for recourse to common law HT, defined in article D312-8 CASF as programmed respite, vacations for the family carer, development of the elderly person’s social integration.

The CHCBE has 2 HTS places.
